To apply for a new or replacement passport for a minor (15 or younger), you will need: |
- Two recent identical color photographs. They must be two inches by two inches (5cm x 5cm) and the size of the head (from hair to chin) must be between .75 inches and 1 inch (2.5 to 3.5cm). The background must be plain and light-colored. For babies, the eyes must be open and there should not be a hand or other head support visible in the photograph.
- A completed DS-11 application form
- The $85 application fee
- Proof of the child’s U.S. citizenship. If replacing a currently valid or recently expired passport, present the current passport as proof of citizenship and identity. If you are applying for a first-time passport or the current passport is unavailable, one of the following is required:
- A certified U.S. birth certificate
- An original Consular Report of Birth Abroad
- An original Certificate of Citizenship or Naturalization
- Evidence of the child’s relationship to parents/guardians (children under age 14), such as one of the documents below. If a parent’s name is different than recorded on the document, evidence of a legal name change is required.
- A Certified U.S. or foreign birth certificate with the parents’ names
- An original Consular Report of Birth Abroad
- An original adoption decree with the adopting parents’ names
- An original or certified copy of a court order establishing custody or guardianship
- Both parents and the child must apply at the Consulate together and in person if the applicant is under 14 years old. Parents must present identification such as a U.S. or foreign passport, a U.S. drivers’ license, U.S. military ID, or Permanent Resident card. If one parent is deceased, an original or certified copy of a death certificate is needed. If a court has awarded sole legal custody to one parent, the original or a certified copy of that order must be presented. If one parent cannot appear at the Consulate, he or she should complete form DS-3053, Statement of Consent. This form needs be notarized in the U.S. or completed at an Embassy or Consulate abroad.
