Lost or Stolen Passport

Replacing a Lost or Stolen Passport

To apply for a replacement of a lost or stolen passport, you will need:

  • Two recent identical color photographs. They must be two inches by two inches (5cm x 5cm) and the size of the head (from hair to chin) must be between .75 inches and 1 inch (2.5 to 3.5cm). The background must be plain and light-colored.
  • The application fee of $97 for adults and $82 for minors (15 or younger)
  • Any documents you have which show your identity and/or U.S. citizenship, such as a driver's license or other photo ID, a birth certificate or a naturalization certificate. The Consulate has computer records that may help determine your eligibility for a U.S. passport, but it is the responsibility of each applicant to prove his or her identity and citizenship.
  • If replacing a stolen passport, a police report, if available
  • If you are replacing a minor (15 or under) passport, please note the two-parent consent requirements for obtaining a minor passport.
